• You will activate your kit exclusively with the TDDP and testing your dog within 10 day or less or receiving your kit or within the first 10 days following birth.
• You agree to swab the dog for which you ordered the kit and submitted documentation for only.
• You understand that we do not recommend nor promote sharing your advanced results with anyone you do not trust implicitly and we do not endorse or promote any other organizations, databases, or programs.
• We will not share personal information provided as part of your form submission or your order but we will use and share data per your form selections.
• The kit prices are subject to change over time. We cannot guarantee our discount remains consistent or that we will not increase/decrease the number of kits needed in order to participate in the program.
• We do not promise an exact turn around timeline. Kit turn around (upon receipt by Embark) Embark has an officially listed turn around time statement that can be found "here". We have seen a typical, correctly collected sample take 2-8 weeks for a full turn around, but it can be shorter or longer depending on any number of factors. These factors include, but are not limited to holidays, ongoing research projects, the collected sample's viability, how many other kits have been returned in/around the same times as yours, etc.
• You will be notified via results@tamaskandogdiversityproject.com to the email provided on your submission form when your kit has been shipped and will receive all kit updates from that email only. You should contact this same email or send a message on our Facebook page with any questions. No other form of communication is officially associated with the project and administrators/ambassadors of the TDDP will prompt you to contact one of those two outlets - they will not respond to questions via other methods of contact.
• You will forfeit your kit(s) if you fail to activate your kit(s) with the TDDP and provide confirmation of the Embark kit(s) return to TDDP/Embark within 15 days of receiving your kit(s)
• Only registered owners and breeders (for future/active litters in whelp) may submit orders for a kit(s) through our program. Payment may be completed by anyone, but no other submissions will be accepted or considered valid.
• Embark may share your name and number of kits ordered through our Embark portal with the TDDP admin team in order to allow us to confirm legitimacy of the orders and that all kits purchased were done so only for approved and eligible participants.  

Data Use

• You accept and understand that the TDDP will be collecting your dog's data directly from Embark and has permission to continue to do this as long as your dog's profile is shared with any of the TDDP Embark profiles.
• Our organization may show data, graphs, etc. both in our private facebook group, on our official facebook page, or on any area of our website/publications. Depending on what options you selected in your order form will dictate if your dog/your identity will be shared or kept anonymous.
• Your kit results will be given to Embark to include in our project and tools (current and future), will be available for their future research and any research projects that the TDDP participates in in order to research the health, development, and future of the breed.
• If you have questions about using this form, the stipulations of use, etc. you may email results@tamaskandogdiversityproject.com. Purchase of these breed-specific kits means that you will agree to abide by these stipulations and understand that a breach of these stipulations may result in billing for the remaining amount of a full priced kit via Embark's current rate if you break this agreement.
• The TDDP retains primary ownership of any and all Embark profiles for which TDDP has sponsored Embark testing. Owners will receive full control to view/edit their dog's Embark profile.

Report & Certificates

• You will be provided with a genetic report by email once we have enough of your dog's kit processed to provide it. The Tamaskan Dog Diversity Project Genetic Report was designed and created exclusively for the TDDP. This report provides coat color genetics, genetic diversity map, genetic diseases associated mutations specific to the Tamaskan Dog breed family, genetic haplotype diversity and genetic COI. 
• If you order through this portal or share your previously tested dog's profile(s) with one of our Embark accounts we will provide a litter prediction. *Both the prospective dam and sire's profiles must be currently shared in order to receive this additional report. At this point in time, only NTC approved litters are eligible for this beta feature.
• For litters who are fully tested through the TDDP as part of our beta breeder ordering option we are offering parentage confirmation certificates (digital and print options). This additional feature does require both the parents of the litter to also be Embark tested with, or before, the litter.

• You are only allowed to purchase a kit for a Tamaskan Dog or Tamaskan outcross registered with one of the following breed registries: Tamaskan Dog Register, Tamaskan Germany, Nederlandse Tamaskan Club, Interessengemeinschaft Tamaskan and International Tamaskan Register. Any and all other dogs are NOT qualified for this project. If your dog does not qualify for the discount and you would like to test with Embark, we encourage you to do so using the following link and to share your profile with us using the directions found here, but we can not include any other dogs in the project.
• The registration information provided in your submission form may be verified with the respective registry (or registries) by a project member to ensure accuracy and authenticity of your registered Tamaskan Dog (or outcross for the Tamaskan Dog breed).
• A lack of registration documentation or any inconsistencies in the validity or accuracy of said documentation may cause your dog's profile to be maintained by the TDDP (while the profile will be accessible and fully manageable by you) until your dog's registration is verified. Please understand this is not only to guarantee the reputability of our research, but also to protect owners and dogs alike from any ill-intentioned testing/participation in our research.
• Dogs registered with the UK Tamaskan Dog Register may utilize the discount as we are attempting to build a genetic profile for dogs of recent Tamaskan Dog heritage to assist with our breed project. At this time these participants may only be offered limited services that the TDDP provides to other accepted registry participants. If at any point in the future the UK TDR dogs are proven through our research to be of validity and value to the project they will be granted all of the same access and benefits of the other participants.

• No staff or other individual officially associated with the TDDP will contact you in any way other than this form to submit, update, or verify results. Only emails from results@tamaskandogdiversityproject.com or messages from our official facebook page can direct and contact you about your order, results, their use, and any additions in the future. Any other contact is not official, not affiliated, or otherwise recommended by this project. We have the safety, privacy, and wellbeing of the breed and your dogs at heart and want to warn you against sharing the data that we are asking for with anyone besides those you trust implicitly.
  
• Embark or TDDP may contact you about your dogs profile or your Embark profile requesting additional information in the future. This may include, but is not limited to: updated results, requests for images, video, data, survey completion, etc., results from other health results and research projects, etc.

Beta Programs

We are currently initiating a number of beta programs into the TDDP. Different programs, forms, processes, etc. will be applicable for different beta programs. The following conditions may apply to one or all of the beta programs.

• You will use results@tamaskandogdiversityproject.com as the contact email for your order via the Embark payment portal.

• You agree to pay your order deposit (as applicable).

• You agree to submit additional health and statistical data as outlined in your submission form.

• All kits ordered by breeders will be retained by the TDDP/Embark until such time as the official registration paperwork is submitted with breeders and owners being granted full access to the profile(s).
• If you do not complete form the profile will be held by the TDDP but full access and ability to edit will be granted to the breeder in your absence.
• If the dog is ever sold or re-homed you will notify the TDDP and ownership of the dog(s) profile will be given to the new registered owner.

Partnered Club Sharing

• Owners of NTC registered dogs are currently eligible to share their dog's results directly with the NTC through their order form. By selecting "share with NTC" in your order, you agree that your dog's profile will be available to the NTC and that they will be able to use your dog in partnered research with the TDDP and Embark. This does not grant them any additional rights nor usage of your dog's information, but allows TDDP to share genetic reports, parentage certificates, and litter prediction information with NTC to expedite any club procedures, quickly confirm registration for TDDP services/eligibility and in order to allow TDDP to better customize your services and tools for your dogs and your club.

Embark Extension
